*Digital signature service is offered by UVa-Wise through Yozons secure document delivery services. Once completing an on-line request, you will receive an email with instructions to retrieve your completed, document from Yozons should you want to download an electronic copy for your own records.

Citizen Tuition Form |
To
be used to request that one letter grade be changed
to another. (DO
NOT USE TO CHANGE INCOMPLETE GRADES TO LETTER GRADES) |

Schedule
Action Form

To
make schedule changes that cannot be made by utilizing
on-line registration (My Uva-Wise). Examples for this
form include: Adding independent study courses, restricted
courses, full courses, etc. (advisor
signature is required). Some schedule
changes require the advisor AND the
Academic Dean's signature. This form replaces
the Class Registration and Schedule Change forms. |
Student
Enrollment Verification
Form |
Used
to prove enrollment status at UVA-Wise. It will include
dates of enrollment, enrollment status (full, part,
half-time), GPA (if requested),
and/or expected date of graduation (if requested).
Please not, enrollment
certification cannot be provided for an upcoming term.
Certification
is only provided for past and current school terms.
Please note: Effective May 31, 2006, this form needs to be completed in order to obtain official enrollment certification. |

may fill-in the necessary form(s) online, print, and obtain
the correct
signatures. All
completed forms may be returned to the Office of the Registrar
for
processing.
