*Digital signature service is offered by UVa-Wise through Yozons secure document delivery services. Once completing an on-line request, you will receive an email with instructions to retrieve your completed, document from Yozons should you want to download an electronic copy for your own records.

Grade Transcript Form

 

Request a copy of grades/academic performance to be sent to another institution, organization, person, etc. Fee Applies

Change of Address Form (PDF Form)

To change a current address.

(please note:  a UVA-Wise campus box cannot be used as a permanent address.)

Declaration of Major Form

To declare a major, change advisor(s), or change/add a major

Course Substitution Form To have a previously completed course substitute for another course that needs to be completed at UVA-Wise for graduation. (Please see your advisor or the Registrar for additional information).
Request to Transfer a Course Form To request permission to transfer a course to UVA-Wise that was (or will be) completed at another community college or university. (signatures are required).
Senior Citizen Tuition Form To be used to request that one letter grade be changed to another. (DO NOT USE TO CHANGE INCOMPLETE GRADES TO LETTER GRADES)

Schedule Action Form

To make schedule changes that cannot be made by utilizing on-line registration (My Uva-Wise).  Examples for this form include: Adding independent study courses, restricted courses, full courses, etc. (advisor signature is required). Some schedule changes require the advisor AND the Academic Dean's signature.  This form replaces the Class Registration and Schedule Change forms.

Student Enrollment Verification

Form

Used to prove enrollment status at UVA-Wise. It will include dates of enrollment, enrollment status (full, part, half-time), GPA (if requested),

and/or expected date of graduation (if requested). Please not, enrollment certification cannot be provided for an upcoming term.

Certification is only provided for past and current school terms.

Please note: Effective May 31, 2006, this form needs to be completed in order to obtain official enrollment certification.

Alumni Graduation Information Sheet  

**Students may fill-in the necessary form(s) online, print, and obtain the correct

signatures. All completed forms may be returned to the Office of the Registrar

for processing.
